What Is Body Tension in Babies?

Body tension in babies is something I see all the time in clinic, especially in babies who are struggling with feeding, unsettled behaviour or sleep.

Most parents don’t come in saying “my baby has body tension.” They’ll usually come to me and say that “something just feels off",” “feeding feels hard and exhausting,” or “my baby is so unsettled after feeds.”

And honestly, they’re usually right.

Because when a baby’s body is holding tension, everything becomes harder than it needs to be, especially feeding.

Common Signs of Body Tension in Babies

Body tension can show up in different ways depending on your baby. Sometimes it’s obvious and sometimes it’s really subtle. But some of the more common signs we see in our clinic, include:

• Arching or stiffening during or after feeds
• Difficulty latching or staying latched
• Clicking or losing suction during feeds
• Preferring one side (breast or bottle)
• A head tilt or signs of Torticollis (tightness through one side of the neck)
• A “C-shaped” or “banana-shaped” body, where your baby curves to one side
• Shoulders bunched up towards the ears
• Little tight fisted hands and arms held tightly in close to the body (I often describe it as “T-rex arms”)
• Red or deep neck creases (in the front and back of the neck) from constant positioning or tension
• Frequent frowning or looking uncomfortable even when not crying

• Unsettled behaviour, especially after feeds
• Short sleep cycles or difficulty settling
• Reflux-like symptoms or discomfort

Not every baby will have all of these.

Sometimes it’s just a few small things that don’t seem like much on their own, but together they start to paint a clearer picture of how your baby’s body is functioning.

How Body Tension Affects Feeding

This is usually the piece that gets missed because feeding isn’t just about getting milk in to your baby. It’s a whole body process for a little one!

For a baby to feed comfortably, they need to be able to relax through their body, move their tongue well and stay organised enough to keep going. However, when there’s tension, they struggle to do all of this. In order to continue feeding, a baby will begin to compensate and work around it.

Sometimes this can look like a baby clamping down on the breast or bottle instead of latching deeply and comfortably. Sometimes a baby will begin pulling on and off, getting frustrated or needing you to keep adjusting them just to stay on or compressing or shaping their breast just so their baby can stay latched on without slipping off.

You might also notice more clicking, more air swallowing or feeds that just feel messy and tiring. When this begins to occur, it often turns into “maybe it’s reflux” or “maybe they’re just a fussy feeder” or “you have low milk supply, you need to top up.”

But quite often, it’s not just a random behaviour your baby has decided to create. It’s your baby’s way to tell you that they’re trying to feed with a body that isn’t moving as easily as it could and that they need help.

When you start to reduce that tension and support how their body is working, feeding usually changes. Feeding will begin to feel calmer, more coordinated, less painful for you and less like something you have to push through.

Where Does Body Tension Come From?

There’s usually not just one reason for a baby experiencing body tension. Most of the time, there’s a few things layered together.

Sometimes it starts before birth. The way a baby is positioned in the womb can create a preference through their body before they’re even born. Then birth itself can add another layer. Whether it was fast, long, induced, assisted or even a really straightforward birth, babies still go through a lot physically.

After that, feeding comes into play. If feeding is already a bit tricky, babies start to compensate and the more they compensate, the more those patterns build.

This is where things like tongue ties can also fit in. If the tongue isn’t moving as freely as it should, babies often use the rest of their body to make up for it or if a baby is crying from an upset tummy from a food protein intolerance, that can also affect a baby’s body and increase tension.

So instead of it being one clear cause, it’s usually a combination of:

  • How they were positioned,

  • How they were born,

  • How they’ve been feeding since.

That’s why just looking at one piece on its own doesn’t always give you the full answer.
